BTQ offers a range of services to help Health Homes, their participating Care Management Providers and their contracted Managed Care Organizations to better manage their patient rosters and successfully navigate New York State reporting and billing processes. 

  • Patient Assignment - BTQ can assist Health Homes with processing and automating the assignment of patients to Care Management Providers based on rosters assigned from New York State and Managed Care Organizations.
  • Maintenance and Submission of Health Home Documentation - BTQ can automate regular activity reporting (tracking submissions) for TCM & Non-TCM patients engaged in outreach and care management services as well as quarterly Care Management Assessment and Reporting Tool (CMART) submissions.
  • Billing - BTQ can generate, submit, and provide reporting on invoicing for NYS Medicaid reimbursement, MCO reimbursement (including Billing Roster File submissions) and Care Management Provider administrative fees.

The BTQ Health Home Portal

Through our secure web portal, users can update specific patients through a simple, user interface or take advantage of batch processes to add patients and update activities using files generated from their own EHR.

For Health Homes using a separate care management application, BTQ’s interface capabilities allow them to take advantage of our program-specific tracking, billing, and reporting capabilities.

Robust, flexible reporting allows users at all levels to quickly understand the status of their patient roster, identify patients requiring updates, and address issues that might impede timely reporting and billing.
